目的 探讨调强放疗联合吉非替尼化疗治疗肺癌的疗效及作用机制.方法 将76例非小细胞肺癌患者根据随机数字表法将入组患者分为两组,每组38例.对照组予以调强放疗治疗,观察组予以吉非替尼化疗同步调强放疗治疗,随访1 a或至患者死亡停止.比较两组治疗总有效率,治疗前后γ-干扰素、肿瘤坏死因子-α、白介素-2水平及不良反应发生状况. 结果 观察组总有效率为86.8%,对照组为60.5%,观察组显著高于对照组(P<0.05).治疗后两组γ-干扰素、白介素-2水平较治疗前显著升高(P<0.05),肿瘤坏死因子-α水平较治疗前显著降低(P<0.05),治疗后两组间比较差异有统计学意义(P<0.05或0.01).观察组不良反应发生率为13.2%,对照组为7.9%,两组比较差异无统计学意义(P>0.05).结论 调强放疗联合吉非替尼化疗治疗肺癌疗效显著,优于单用调强放疗,可能通过上调γ-干扰素、白介素-2水平,下调肿瘤坏死因子-α水平发挥作用,不良反应轻,值得在临床推广应用.、Objective To explore the curative effects and action mechanism of intensity modulated radiation therapy (IMRT) plus gefitinib chemotherapy for lung cancer.Methods Seventy-six non-small cell lung cancer patients were assigned to two groups of 38 ones each according to random number table.Control group was treated with IMRT, observation with gefitinib chemotherapy plus IMRT, and followed up for 1 year or up to patients‘ death.Effective rate, as pre-and post-treatment IFN-γ, TNF-α and IL-2 levels as well as adverse reactions were compared between two groups.Results The total effective rate was respectively 86.8% in observation and 60.5% in control group and the former significantly higher than the latter (P<0.05).After treatment IFN-γ and IL-2 levels of both groups heightened more significantly (P<0.05), TNF-α lowered (P<0.05), group differences were significant (P<0.05 or 0.01).The incidence of adverse reactions was 13.2% in observation and 7.9% in control group, group difference wasn‘t significant (P>0.05).Conclusion IMRT plus gefitinib chemotherapy has an evident effect compared with single IMRT for lung cancer, play a role via up-regulating IFN-γ and IL-2 levels and down-regulating TNF-α possibly, has mild adverse reactions, and deserves clinical generalization and application.
